Refractory Anemia: Implications for the Anemia of Chronic Disease.
Clonal SA is related to various MDS subcategories including
refractory anemia with ring sideroblasts (RARS), refractory cytopenias with multilineage dysplasia with ring sideroblasts (RCMD-RS) and
refractory anemia with ring sideroblasts and thrombocytosis (RARS-T).
14 patients had
refractory anemia (RA), six had an isolated deletion of the long arm of chromosome 5 (del(5q)), 5 had
refractory anemia with ring sideroblasts (RARS), 17 had refractory cytopenia with multilineage dysplasia (RCMD), 15 had
refractory anemia with excess blasts 1 (RAEB-1), and 19 had RAEB-2.
Refractory anemia was present in 66.6% patients in NDCD group and 41.1% patients in CCD group.

(1) The 88 patient population consisted of 58 men (65.9%) and 30 women (34.1%) (median age, 62 years; range, 22-87 years); 10 patients (11.4%) had refractory cytopenia with unilineage dysplasia (RCUD), 17 (19.3%) had refractory cytopenia with multilineage dysplasia (RCMD), 28 (31.8%) had
refractory anemia with excess of blasts-1 (RAEB-1), 25 (28.4%) had RAEB-2, 1 (1.1%) had
refractory anemia with ring sideroblasts, and 7 (8.0%) had MDS-unclassifiable.
The different malignancies that caused pancytopenia in their study were AML (62.5%), acute lymphocytic leukemia (28.12%), plasma cell myeloma (3.12%), non-Hodgkin's lymphoma (3.12%), and myelodysplastic syndrome (i.e.,
refractory anemia with excess of blasts, 2 (3.12%)].

The findings are consistent with a highgrade myelodysplastic syndrome, best classified as
refractory anemia with excess blasts-2 (RAEB-2) on the basis of the blast count and presence of myelodysplasia.
Siddappa and colleagues presented a rare case of renal hemosiderosis appearing with renal failure due to repeated blood transfusion (about 2 per month) over past 2 years for chronic
refractory anemia.[sup.25] Three cases of paroxysmal nocturnal hemoglobinuria with renal hemosiderosis and renal failure were reported by Nair and colleagues.[sup.10] Most patients with renal hemosiderosis also have renal failure.
