Unusually large canker sores Recurring sores, with new ones developing before old ones heal, or frequent outbreaks Persistent sores, lasting two weeks or more Sores that extend into the lips themselves (vermilion border) Pain that you can't control with self-care measures Extreme difficulty eating or drinking High fever along with canker sores See your dentist if you have sharp tooth surfaces or dental appliances that seem to trigger the sores.

5 Regarding pigmented lesion on the lip, shave biopsy is acceptable for a lesion that has not crossed the vermilion border. If the pigmented lesion has crossed vermilion border, a punch biopsy is preferred aligned vertically and the margins must be painted with gentian violet to avoid misalignment while closing the defect.6

Consequently, the lip border (vermilion border) becomes less clear and the lips may gradually become scaly and indurated.

Other characteristic oral findings include the reduced vertical dimension of the lower face, disappearance of the vermilion border, malformed teeth, the oral mucosa becomes dry, xerostomia and the lips become prominent.
